New Patient Leaflet

The doctors welcome new patients who live within our practice area.

To register as a patient please complete an application form. These are available at the Reception desk. You will be asked to supply recent proof of address i.e. a utility bill, bank statement or driving licence.

As it often takes some time for records to be forwarded from your former practice, all newly registered patients will be asked to complete a health questionnaire:

Should you be on repeat medication, you will need to see a GP before any medicines can be issued. We therefore suggest you ensure you have enough from your previous GP to last until your appointment here.

Medical treatment is available from the date of registration. Please contact Reception for further information.
